Your cooperation in complying with the following guidelines is much appreciated. We are concerned with your child’s oral health, as well as taking your schedule into consideration. We believe these guidelines will best benefit all of our patients and families.

Appointment Times:

Our office makes every attempt to schedule appointments at times that are convenient for you and your family. However, in a practice limited to the care of children and teenagers, all appointments cannot be made after school hours. Children are fresher and have more resources for coping earlier in the day. Therefore, certain morning and early afternoon appointments are necessary. We realize that this may inconvenience some parents on occasion, but it allows us to provide the highest quality of care.

Appointment Reminders:

As a courtesy, our office will attempt to confirm your upcoming appointment 1-2 days beforehand.

Cancelled Appointments:

If you cannot keep your appointed time, please contact our office by phone or e-mail at least 24 – 48 hours before your scheduled time so that we can continue to provide quality care on a timely basis, as well as utilize the reserved time for those children who need urgent care.

Missed Appointments:

Our office reserves the right to charge for a missed appointment or one in which the courtesy of a phone call and advanced notice is not given (Ranging from $50 – $150 – depending on the type and length of time reserved for the appointment).

Financial Policy:

Payment for services are due at the time of the visit unless other arrangements have been made prior to the scheduled appointment. We accept most traditional insurance policies and are network providers for Aetna PPO, Assurant PPO, Blue Cross Blue Shield PPO, Cigna PPO – Radius Network, CompBenefits PPO, Connection Dental PPO, Delta Dental Premier/PPO, Dentemax PPO, DHA-PPO, Guardian PPO, Humana PPO, MetLife PDP/PPO, Principal PPO, United Concordia-Alliance and United Healthcare PPO and many more. Patients with dental insurance must make payment for the portion of treatment costs that are not covered by the insurance at the time of treatment. We accept cash, checks, Visa, MasterCard and Discover. Additionally, we offer no-interest financing through CareCredit or our in-office automatic credit/debit card payment plan. We will provide you with an ESTIMATE for treatment (time permitting) and anything not covered by your insurance carrier is your responsibility.

*In an effort to keep our fees reasonable during these economically challenging times, there will be a $12 per month billing charge for any account over 60 days past due and has not made prior payment arrangements.
